I updated everything and turn everything off while booting back into Windows 11 and still nothing!
Update all drivers (GPU, audio, chipset, etc.), windows updates, DirectX, and necessary software. I recommend doing a clean install of drivers after using DDU (Display Driver Uninstaller). This fixes common driver issues that can affect various games. Also, try disabling any overclocks and things running in the background like Afterburner, overlay programs, anti-virus, etc. or doing a clean boot of Windows 11.

That way you can find out if it is something specific to your system configuration that is causing the issue and correct it.
Other than that, I recommend visiting the game's official forums or Reddit to see if this is a common issue that others are experiencing and they may have solutions for you.
Do you get any error? have you checked event viewer? what release do you use to play the game, is it GoG or Steam?
I have it on Steam, and it's the only game I'm having any problems with.
@power_of_tien: Which version of the game are you running Original or Next gen update?
I'm assuming its the Next Gen version.
Have you totally uninstalled the game and reinstalled it? Next thing i can suggest is disable geforce experience. Also can you select the dx11 version of the game when you launch it or is there a safe mode option?
I don't know what version I have, I just bought the 4090, 3 days ago and I never had a problem Witcher 3 b4, I had game since it came out I think.

@power_of_tien: Are you trying to run the game through GOG Galaxy or through it's own shortcut/.exe?
Try uninstalling GOG Galaxy and downloading the latest version. Auto updates stopped working for me and I had to do this before.
just through Steam and the shortcut.exe.. I've never used GOG and don't know what that even is LOL/
